Description

This presentation provides an overview of exosomes as an emerging modality in regenerative dermatology, with a particular focus on hair restoration. Exosomes—small extracellular vesicles measuring 30–150 nm—are naturally present in multiple biological fluids and function as carriers of proteins, lipids, and nucleic acids, enabling intercellular communication. Once regarded merely as waste-transport particles, they are now recognized for their therapeutic, regenerative, and diagnostic potential, reflected by a rapid increase in scientific publications and a multibillion-dollar industry growth. The lecture outlines the mechanisms by which exosomes promote hair growth, notably through activation of the β-catenin signaling pathway, which prolongs anagen, enhances follicular regeneration, and improves dermal–epidermal communication. Evidence from recent clinical studies—including systematic reviews published in 2023 and 2025—shows consistent improvements in hair density and thickness across a variety of exosome sources such as adipose tissue, placenta, umbilical cord, bone marrow, and hair follicles, with high patient satisfaction and excellent short-term safety. A practical framework is presented for selecting high-quality exosome products, highlighting five essential questions regarding source, donor screening, isolation method, purity, and supporting studies, along with the relevance of GMP certification. Clinical examples demonstrate exosomes used alone or in combination with other treatments, showing promising outcomes in androgenetic alopecia. The presentation concludes that exosomes represent a promising frontier in regenerative hair therapies. While early data are encouraging, standardized protocols, long-term safety data, and regulatory oversight remain critical for future integration into routine dermatologic practice.

Piotr Krajewski, MD, PhD, is an Assistant and Research Fellow at the Division of Dermatology, Venereology and Clinical Immunology at Wrocław University of Science and Technology. He also works as a Guest Scientist at the Lübeck Institute of Experimental Dermatology at the University of Lübeck in Germany. Dr. Krajewski obtained his medical degree from Wrocław Medical University in 2019, including a year abroad at the University of Málaga in Spain through the Erasmus program. He completed his PhD at Wrocław Medical University’s Department of Dermatology, Venereology, and Allergology in 2023, and he became board-certified in Dermatology and Venereology in May 2025. Additionally, he earned a Master’s Degree in Trichology and Hair Transplant from the University of Alcalá in Madrid between 2023 and 2024. His research interests revolve around chronic skin conditions like hidradenitis suppurativa, as well as psychodermatology and the neurophysiology of itch. He’s published over 130 peer-reviewed articles, which have accumulated a total impact factor of 381.77 and nearly 1,000 citations. His work is recognized for its significance, with an h-index of 17 on Google Scholar, 18 on ResearchGate, and 15 on Web of Science. He also serves as a Guest Editor for several international journals. Dr. Krajewski has received several notable awards, such as being a finalist in the 2021 Polityka Science Awards, winning the 2023 START Scholarship from the Foundation for Polish Science, and receiving the 2024 Golden Otis Award of Trust.

He holds a PhD and MSc in Medicine from the State University of Rio de Janeiro (UERJ), where he also served as Professor of Dermatology at the University Hospital Pedro Ernesto from 2017 to 2024. He completed his postgraduate training in Dermatology at the Federal University of the State of Rio de Janeiro (UNIRIO). Dr. Melo is a board-certified dermatologist by the Brazilian Society of Dermatology (SBD) and completed advanced international fellowships in Hair Diseases at the Universities of Bologna/Modena (Italy) and Miami (USA). His leadership includes serving as Head of the Hair Department of the SBD regional Rio de Janeiro (2015–2018) and later as National Coordinator of the Hair Department of the Brazilian Society of Dermatology (2023–2024). He has authored approximately 100 scientific papers and 10 book chapters in trichology, contributing significantly to the understanding and management of complex scalp disorders such as scarring and non-scarring alopecias. His work emphasizes the integration of trichoscopy and novel therapeutic approaches, bridging clinical research and patient-centered care. Dr. Melo is an active member of several international dermatologic societies, including the International Trichoscopy Society (ITS), the European Hair Research Society (EHRS), the American Hair Research Society (AHRS), the American Academy of Dermatology (AAD), the Ibero-Latin American College of Dermatology (CILAD), and the Brazilian Society of Dermatology (SBD). In 2023, he was awarded the Young Dermatologist Award at the World Congress of Dermatology in Singapore, recognizing his innovative contributions to hair research and clinical dermatology.

Dr. Leonardo Spagnol Abraham is a board-certified dermatologist specialized in hair and scalp disorders. He completed his dermatology residency at the “Instituto de Dermatologia Prof. Rubem David Azulay” in Rio de Janeiro, Brazil, where he later co-founded and coordinated the Trichology Clinic from 2007 to 2012. The ear, with its thin skin and sebaceous gland concentration, reveals specific vascular and follicular clues important in distinguishing benign from malignant lesions.

On the chest and back, where the skin is thicker and sun exposure varies, dermoscopy must account for irregular pigment distribution and architectural disorder, especially in large nevi or early melanomas. Limb lesions may show distinctive features due to mechanical friction, hair density, and vascular variations, demanding precise interpretation to identify atypical nevi or skin cancers.

Palmar and plantar dermoscopy highlights the parallel ridge pattern critical for melanoma diagnosis, contrasting with benign acral patterns like the parallel furrow or lattice-like structures. Scalp and hair disorders benefit from trichoscopy, where dermoscopic evaluation reveals specific signs such as yellow dots, broken hairs, or black dots, aiding in the diagnosis of alopecia areata, androgenetic alopecia, or tinea capitis.

In nail disorders, onychoscopy enables visualization of melanonychia, hemorrhages, and nail matrix changes, crucial for distinguishing subungual melanoma from benign causes like trauma or fungal infection. Mucosal dermoscopy, though technically challenging, provides diagnostic clues in pigmented lesions of the lips, genitalia, or oral mucosa, requiring adaptation to moist, non-keratinized surfaces.

Finally, ultraviolet dermoscopy reveals a unique application: scabies mites fluorescing bright green under UV light, enhancing detection when traditional visualization fails. Topographic dermoscopy thus demands both anatomical knowledge and technical adaptation to maximize diagnostic accuracy across diverse body sites.

Dermatological diseases, particularly neglected tropical diseases (NTDs) with skin manifestations like deep mycosis (chromoblastomycosis, sporotrichosis, mycetoma), scabies, leprosy, lymphatic filariasis, and cutaneous leishmaniasis, pose major challenges for healthcare systems in resource-limited regions of Africa, Asia, and Latin America. These conditions severely affect vulnerable populations, suffering from frequent underdiagnosis and inadequate treatment that exacerbates suffering. Diseases such as atopic dermatitis are also under consideration for inclusion as skin NTDs through collaborative efforts involving ISAD, ASDV, and WHO. Furthermore, albinism, highly prevalent in sub-Saharan Africa, presents significant social challenges including stigmatization and occult beliefs. Despite these complex difficulties, the field is undergoing a historic transformation driven by science and technology, particularly artificial intelligence (AI), which offers tangible tools for improving diagnosis, treatment, and prevention. The participation of global experts facilitates vital knowledge exchange, exploration of innovative solutions, and helps address critical shortages of human and material resources in remote areas.
